How to apply: The application should be submitted at any one of the 99 designated Jeevan Centres situated all over Delhi between 8:00 a.m to 5:00 p.m on payment of Rs. 20/- (non refundable) by cash. (To locate the nearest Jeevan Centre, applicants may call on the toll free no. 1800-180-2039 or visit the Delhi Government website. By post to DSHM office/By Hand in the drop box at DSHM Office address as given above- along with postal order of Rs. 20/- (non refundable) payable to Delhi State Health Mission.
